Qualifications

Current Registered Nurse (RN) license in New Jersey is required.
2+ years of clinical nursing experience in a long-term care (LTC) setting.
Proven experience with MDS 3.0 completion, RAI processes, and Medicare/Medicaid reimbursement.

Responsibilities

Coordinate the interdisciplinary team to complete MDS assessments and Care Plans.
Track Medicare eligibility and perform concurrent reviews for accuracy.
Oversee the electronic transmission of MDS data to state and federal agencies.
Lead daily admission and care planning meetings.
Conduct regular audits of medical records to ensure accurate documentation.
Train facility staff on MDS terminology and documentation requirements.
