Job summary

Responsibilities include ensuring accurate documentation, standing and walking as needed, and working with physicians and staff to support residents throughout their stay.

Qualifications

• RN licensed in the state you will be working
• Two years of experience with long-term care plans, MDS state regulations, and CMS related to MDS preferred
• Excellent communication skills to interact with residents, visitors, and staff
• Ability to stand and walk for long periods; ability to lift heavy objects with proper lifting techniques

Responsibilities

• Completes all MDS, admission, significant change, change of therapy, end of therapy, discharge and re-entry tracking forms.
• Transmits MDS data into CMS via the state agency according to CMS timelines.
• Ensures all MDS/CAA records are available for a fifteen month period.
• Collaborates with the interdisciplinary team to develop and implement annual care plan goals.
• Develops and maintains an effective care plan process for residents' stay.
