• Coordinate individualized plans of care using an interdisciplinary team approach.
• Review and evaluate resident MDS assessments and care plans to ensure proper implementation.
• Participate in resident care plan conferences.
• Maintain the MDS schedule within required time frames and communicate pending assessments to relevant directors.
• Generate and distribute the monthly MDS calendar.
• Complete and audit MDS assessments, ensuring all required signatures are obtained.
• Transmit electronic copies of assessments to the Centers for Medicare and Medicaid Services (CMS) and reconcile all transmissions.

What We're Looking For

• Current RN license in the State of Maryland.
• MDS certification or proven experience completing MDS assessments.
• Minimum 2 years of clinical experience. Minimum of 1 year of experience in a long‑term care setting required.
• Prior experience as an MDS coordinator is strongly preferred.
• Must possess strong written and oral communication skills; able to maintain confidential information.
• Must be proficient in Microsoft Office.
• Strong organizational and analytical skills with excellent attention to detail.
