Job summary

Lakeside Health & Rehab in Richmond, VA, is seeking an experienced RN MDS Coordinator to join our clinical leadership team. This role focuses on accurate assessments, comprehensive documentation, and ensuring PDPM reimbursement supports high-quality resident care.

As MDS Coordinator, you will lead the MDS process, coordinate with nursing, therapy, social services, and dietary staff, and stay current with CMS and RAI requirements.

Qualifications

• Active Registered Nurse license in Virginia is REQUIRED.
• MDS experience strongly preferred.
• Experience in skilled nursing/long-term care is preferred.
• Strong knowledge of MDS/RAI guidelines and CMS requirements.
• Excellent attention to detail and organizational skills.

Responsibilities

• Complete accurate MDS assessments per RAI and CMS guidelines.
• Coordinate MDS process and ensure timely completion.
• Lead interdisciplinary care planning.
• Review documentation to support coding and PDPM reimbursement.
• Collaborate with nursing, therapy, social services, dietary, and more.
• Monitor quality measures and support quality improvement.
• Educate staff on best practices and documentation.
• Ensure care plans reflect residents' needs and conditions.
• Stay current on regulatory requirements and survey readiness.

Lakeside Health & Rehab is seeking an RN MDS Coordinator to join our clinical leadership team! This is an excellent opportunity for an RN who understands the importance of accurate assessments, strong documentation, regulatory compliance, and interdisciplinary collaboration in a skilled nursing environment.

As our RN MDS Coordinator, you’ll play an essential role in connecting clinical care, documentation, quality outcomes, and reimbursement—all while helping ensure our residents receive the individualized care they deserve.

What You'll Do

• Complete accurate and timely MDS assessments in accordance with RAI and CMS guidelines
• Coordinate the MDS process and ensure assessments are completed within required timeframes
• Lead and participate in interdisciplinary care planning
• Review clinical documentation to support accurate coding and PDPM reimbursement
• Collaborate with nursing, therapy, social services, dietary, and other departments
• Monitor quality measures and support facility quality improvement initiatives
• Identify documentation opportunities and educate nursing staff on best practices
• Ensure resident care plans accurately reflect individual needs, goals, and clinical conditions
• Stay current on MDS, RAI, CMS, and regulatory requirements
• Support survey readiness and ongoing regulatory compliance

What We're Looking For

• Active Registered Nurse (RN) license in Virginia – REQUIRED
• Previous MDS experience strongly preferred
• Skilled nursing/long-term care experience preferred
• Strong understanding of MDS, RAI guidelines, PDPM, and care planning
• Knowledge of CMS requirements and skilled nursing documentation practices
• Excellent attention to detail and organizational skills
• Strong clinical assessment and critical-thinking abilities
• Ability to effectively collaborate with clinical and interdisciplinary teams
• MDS/RAI certification preferred or willingness to obtain
