Avardis Health is seeking a qualified Registered Nurse (RN) to serve as an MDS Coordinator in Ashland. The role oversees the Resident Assessment Instrument (RAI) process, ensuring accurate assessments for optimal care planning and Medicare/Managed Care reimbursement.

The position calls for detail-oriented, compliant professionals who thrive in a structured care setting. The ideal candidate will have an active RN license, 6+ months of MDS Coordinator experience, and preferred RAC-CT or RNAC

Qualifications

  • Active RN license in good standing.
  • 6+ months of experience as an MDS Coordinator.
  • Experience in Skilled Nursing/Rehabilitation preferred.
  • RAC-CT or RNAC certification preferred.

Responsibilities

  • Coordinate and complete the MDS, CAA’s, and Care Plans in compliance with Federal and State regulations.
  • Monitor and document management of Medicare and Managed Care residents with interdisciplinary team.
  • Ensure accuracy in resident assessments to maximize reimbursement and quality of care.
  • Drive process improvements to enhance resident outcomes.
  • Maintain compliance with evolving regulations and best practices.

Major Responsibilities

  • Monitor and document the management of Medicare and Managed Care residents, collaborating with the interdisciplinary team.
  • Ensure accuracy in resident assessments to maximize reimbursement and provide high-quality care.
  • Drive innovation by implementing new ideas and processes to improve resident outcomes.
  • Maintain compliance with evolving regulations and best practices.
Minimum Qualifications
  • Active RN License in good standing.
  • 6+ months of experience as an MDS Coordinator required.
  • Experience in Skilled Nursing/Rehabilitation preferred.
  • RAC-CT or RNAC certification preferred.
