Job description

RN MDS Coordinator (Finance)

Responsible for conducting and coordinating the development and completion of resident assessments, in accordance with State, Federal, and Company guidelines. Oversees the generation of Minimum Data Set (MDS) for each Medicare patient and ensures electronic transmission of required data within mandated timeframes.

Key responsibilities include:

  1. Coordinating the development and completion of resident assessments (MDS), including implementation of CAAs and Triggers, conducting or coordinating resident interviews, evaluating medical data, and ensuring timely completion and transmission.
  2. Assisting community directors and supervisors in scheduling assessment/care plan meetings, and coordinating participation of outside team members and family members.
  3. Developing and maintaining written care plans that identify resident needs, care requirements, goals, and responsible professionals, with measurable objectives and timetables.
  4. Maintaining policies and procedures related to MDS and care plans, and ensuring availability of the MDS Instructor's Manual.
  5. Implementing and monitoring a quality assurance program for assessments and care plans, and reviewing QI and QM reports for corrective actions.
  6. Supporting discharge planning and participating in community surveys and inspections.
  7. Providing leadership in interdisciplinary committees and developing training programs on assessment skills and techniques.

This position requires Medicare/Medicaid experience and offers opportunities for professional growth, with comprehensive benefits including health insurance, retirement plans, paid time off, and other employee programs. Salary range is $33.88 to $43.15 per hour, based on experience.
