Description
Responsible for conducting and coordinating the development and completion of resident assessments, in accordance with the requirements of State, Federal, and Company guidelines. Responsible for overseeing the generation of Minimum Data Set (MDS) for each Medicare patient and electronic transmission of required data within the mandated time frame.
Coordinates the development and completion of the resident assessment (MDS) in accordance with current regulations and guidelines, including the implementation of CAAs and Triggers; conducting or coordinating the interview(s) of each resident for the residents assessment; evaluating each resident's condition and pertinent medical data; developing and implementing procedures with the Director, Clinical Services for the arrival of newly admitted residents; ensuring all assessments are completed and transmitted timely; assisting community directors and supervisors in scheduling resident assessment/care plan meetings; and contacting and assisting in scheduling participation by outside members of the care team, including residents' representatives and family members.
Coordinates the development of written care plans (preliminary and comprehensive) for each resident that identify problems/needs, care requirements, goals, and responsible professionals. Ensures the care plan includes measurable objectives and timetables to meet residents' needs as identified in assessments.
Ensures the generation and transmission of MDS is complete and timely. Maintains and updates policies and procedures governing resident assessments and care plans. Ensures current copies of the MDS Instructors Manual are available to staff completing MDS portions.
Develops, implements, and maintains an ongoing quality assurance program for resident assessments and care plans. Monitors community QI and QM reports to facilitate appropriate corrective actions.
Assists the resident and Discharge Planning Coordinator in completing the care plan portion of the discharge plan. Participates in discharge planning functions as necessary.
Participates in community surveys (inspections) by authorized government agencies. Provides leadership and participates in committees such as Interdisciplinary Care Plan Team, Policy Advisory, and Quality Assessment and Assurance, providing reports and updates as needed.
Develops and participates in planning, conducting, and scheduling in-service training related to assessment skills and techniques, including interviewing, rehabilitation principles, psychotropic drugs, and care plan functions. Attends annual community in-service training programs.
