Base pay in range will be determined by applicant’s skills and experience. Role is also eligible for team-based bonus opportunities. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.

Responsibilities
  • Coordinate the development and completion of resident assessments according to State, Federal, and Company guidelines, including the generation of MDS for each Medicare patient and electronic transmission within mandated timeframes.
  • Develop and implement resident care plans, ensuring they include measurable objectives and timetables.
  • Maintain policies and procedures related to resident assessments and care plans.
  • Develop and monitor quality assurance programs for assessments and care plans.
  • Assist in discharge planning and participate in community surveys and various committees.
  • Develop and conduct training on assessment skills and techniques, including initial and refresher courses.
Qualifications

Must possess a Nursing Degree from an accredited college or university. Minimum two (2) years supervisory experience in a healthcare setting, and at least six (6) months training in rehabilitative and restorative nursing. Knowledge of long-term care regulations and guidelines. Current State RN license.

  • Standing, walking, sitting, use of hands and fingers, reaching, and potential exposure to communicable diseases, latex, blood-borne pathogens, and physical injury risks.
