Job summary

Avina Healthcare is seeking an MDS Coordinator to join our nursing leadership team. You will assess and monitor resident treatment, determine health status, care plans, and intake procedures in accordance with state and federal standards.

This role requires focus on accurate MDS coding and care coordination. Ideal candidates have 1 year of MDS Coordinator experience, Wisconsin nursing licensure in good standing, and proficiency with MS Outlook/Office.

Qualifications

• Must have 1 year experience as an MDS Coordinator.
• Must have valid and current nursing license in good standing with Wisconsin.
• RN preferred.
• Proficient with computers and systems such as Microsoft Outlook and Office.
• PCC knowledge and experience is preferred.

Responsibilities

• Understand MDS 3.0 terminology to master the MDS process and reimbursement.
• Execute accurate and timely completion of Medicare/Medicaid MDS processes for reimbursement.
• Use strong assessment skills and be detail oriented with MDS completion.
• Coordinate with the Interdisciplinary Team to address resident programming and MDS compliance.
