Lincoln Park Nursing and Rehab in Milwaukee is seeking a dedicated MDS Coordinator to oversee patient care plans and coordinate services to support timely recoveries. You will educate patients and families on prevention and healthy habits while adhering to safety guidelines.

The ideal candidate has 1–2 years of nursing or MDS coordination experience, strong communication, and superb organizational skills to manage multiple tasks in a busy facility.

Responsibilities

• Monitor patient’s condition and assess needs to provide the best possible care.

• Adjust and administer medications per physician’s orders.
• Inspect facilities to maintain hygiene and safety.

Responsibilities:MDS Coordinator

• Expand knowledge and capabilities by attending educational workshops, conferences etc.

Requirements:MDS Coordinator

• A minimum of 1-2 years’ experience

• A team player with excellent communication and interpersonal skills

• Outstanding organizational and multi-tasking skills

• Valid nursing license in the state of Wisconsin
