Job summary

AA Healthcare Management in Fond du Lac, WI, seeks an MDS Coordinator to join the nursing leadership team. You will assess and monitor resident care plans, determine health status, and ensure compliance with state and federal standards during admissions and ongoing MDS processes.

The role requires an active nursing license (RN preferred, LPN acceptable) and at least 1 year of MDS experience. Initially on-site for 90 days, with a possible 3-on/2-off hybrid schedule based on performance and needs.

Qualifications

• Must have 1 year experience as an MDS Coordinator.
• Must hold a valid nursing license in good standing with Wisconsin.
• Role requires on-site presence at the facility for 90 days; hybrid 3 days on-site / 2 days remote may follow based on performance.

Responsibilities

• Understand MDS 3.0 terminology and the full MDS process and reimbursement system.
• Execute accurate and timely completion of Medicare/Medicaid MDS processes for proper reimbursement.
• Use strong assessment skills and be detail-oriented with MDS completion.
• Coordinate with the Interdisciplinary Team to ensure MDS compliance and appropriate programming.

MDS Coordinator Job Qualifications:

• Must have 1 year experience as an MDS Coordinator.

• Must have valid and current nursing license, in good standing with the state of Wisconsin.

• This role requires on-site presence at our facility for the first 90 days. After this initial period, a hybrid schedule of 3 days on-site and 2 days remote may be available, based on performance and business needs.

• Licensed Practical Nurse (LPN) is acceptable, Registered Nurse (RN) preferred.

• Proficient with use of computers and computer systems such as Microsoft Outlook and Office.

• PCC knowledge and experience is preferred.
