Job summary

Lincoln Park Nursing and Rehab in Milwaukee, WI seeks an MDS Coordinator to care for our senior residents and coordinate comprehensive assessments. You will educate patients and families on prevention and healthy habits, while ensuring high-quality nursing care with minimal supervision.

The ideal candidate holds an active Wisconsin RN license, brings professionalism, and is ready to collaborate with the care team to promote patient recovery and wellbeing.

Qualifications

Responsibilities

• Monitor patient's condition and assess needs; provide best possible care and advice.
• Observe and interpret patient's symptoms; communicate with physicians.
• Collaborate with physicians and nurses to devise individualized care plans; perform routine procedures.
• Adjust and administer patient's medications and provide treatments per physician orders.
• Inspect facilities; maintain hygiene and safety.
• Supervise and train LPNs and nursing assistants.
• Expand knowledge by attending educational workshops and conferences.

You will also be responsible for educating them and their families on prevention and healthy habits. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. You will be able to follow health and safety guidelines faithfully and consistently. The goal is to-promote patient's being-by providing high quality nursing care.

Responsibilities

Requirements

• A minimum of 1-2 years' experience
