Job summary

Avantara Lincoln Park is seeking a dedicated MDS/Clinical Coordinator RN to lead MDS processes and care planning for Guests in a premier Lincoln Park facility. You will coordinate the written plan of care, review nursing notes, and lead QA/QAPI initiatives to ensure regulatory compliance and high-quality resident care.

Qualifications

• Graduate from an accredited nursing program.
• Valid, unencumbered RN license in the state of practice.
• CPR certification.

Responsibilities

• Keep abreast of current federal and state regulations and professional standards.
• Assist with development of comprehensive care plans in coordination with MDS.
• Coordinate, manage and monitor the written plan of care for each Guest.
• Assist nursing management with discharge processes and with review of nursing notes.
• Ensure MDS documentation accurately represents Guests and meets regulatory requirements.
• Participate in QAPI efforts and attend trainings to build QAPI capacity.
