Job summary

Avante at Orlando Skilled Nursing and Rehabilitation Center is seeking an MDS Coordinator to oversee resident assessments and ensure compliance with federal, state, and local regulations. You will coordinate MDS assessments and care plans while collaborating with the interdisciplinary team.

The ideal candidate is a licensed nurse (LPN/RN) with 2+ years of experience, strong regulatory knowledge, and a passion for quality patient care.

Qualifications

• Licensed nurse (LPN/RN) with current license in the state.
• Nursing degree or diploma from an accredited program.
• 2+ years of experience in a hospital, skilled nursing, or healthcare facility preferred.
• Strong knowledge of nursing practices, medical procedures, and regulatory guidelines.
• Leadership and collaboration with interdisciplinary teams.

Responsibilities

• Conduct and coordinate MDS assessments and care planning in compliance with regulations.
• Ensure timely and accurate submission of MDS assessments to the State Repository per RAI guidelines.
• Collaborate with the Interdisciplinary Care Team to determine assessment review dates.
• Evaluate and update resident care plans reflecting health status changes.
• Monitor and analyze Quality Measures Reports to maintain high Five-Star Ratings.
• Educate and collaborate with nursing staff, residents, and families on care plans.
• Participate in facility surveys and regulatory inspections.
• Maintain confidentiality and uphold patient privacy.
