Job summary

Avantara Lincoln Park in Chicago is seeking a Registered Nurse with MDS coordination responsibilities to lead the MDS processes and utilization management. You will ensure compliant care plans, accurate documentation, and effective coordination with the clinical team to support guest-centered care.

The role emphasizes leadership in QAPI initiatives and ongoing compliance trainings, with rotating shifts in a 24/7 health care facility environment.

Qualifications

• Graduate from an accredited nursing program.

Responsibilities

• Keep abreast of current federal and state regulations, as well as professional standards.
• Assist with the development of comprehensive care plans for Guests in coordination with the MDS that accurately addresses the needs of the Guest.
• Coordinate, manage and monitor the written plan of care for each Guest of the facility that identifies the needs of the Guest and goals to be accomplished for each need.
• Assist nursing management with the coordination, management, and review of nurse’s notes to determine if the care plan is being followed.
• Monitor Guest status changes to ensure appropriate and timely nursing or clinical team involvement.
• Assure MDS and support documentation are accurate representation of the Guest and meet regulatory and auditor requirements.
• Perform regular audits of documentation to assure accuracy.
• Assist nursing management with the discharge process.
• Perform administrative requirements, such as completing necessary forms and reports.
• Assure that established infection control and prevention practices and standard precautions are maintained at all times.

Education
