The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking a knowledgeable MDS Assessor (RN) to join our clinical leadership team. You will coordinate MDS assessments,CAAs, and care plans to ensure compliant documentation and accurate reimbursement for residents.

This role requires an RN with MDS experience and a strong aptitude for PDPM, CMS regulations, and interdisciplinary collaboration. Join a team dedicated to quality care, continuous improvement, and regulatory excellence.

Qualificações

• RN license in good standing (New York).
• MDS experience in a Skilled Nursing Facility preferred.
• Strong knowledge of PDPM, CMS regulations, and care planning.
• Excellent organizational, analytical, and communication skills.
• Ability to collaborate with interdisciplinary teams.

Responsabilidades

• Complete and coordinate MDS assessments, CAAs, and care plans per CMS/state guidelines.
• Ensure accuracy and timeliness of MDS submissions and documentation.
• Collaborate with interdisciplinary team to implement individualized care plans.
• Monitor PDPM compliance and regulatory standards.
• Participate in Medicare meetings, care plan reviews, and audits.
• Review clinical documentation to support coding and reimbursement.
• Support quality measures and performance improvement initiatives.
• Stay current with regulatory updates and MDS best practices.
