Qualifications

• Registered Nurse with current, active license in state of practice.
• Minimum of one (1) year of experience in a long term care setting.
• Training program available for RN candidates with demonstrated assessment skills.

Responsibilities

• Oversee accurate and thorough completion of the MDS, CAAs and Care Plans in accordance with regulations.
• Act as in-house Case Manager coordinating care with physicians, families and payers.
• Identify regulatory risk and communicate with Nursing leadership to optimize outcomes.
• Capture accurate ARD and RUG scores with therapy and IDT collaboration.
• Ensure timely submission of MDS to state data base and review validation reports.
• Educate IDT and new staff on RAI processes and MDS requirements.
• Analyze QA/QM data to identify trends and support continuous improvement.
• Participate in committees (QA&A) as required by administration.
