Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset is actively seeking an experienced MDS Assessor (RN) to lead MDS processes and ensure accurate documentation for optimal reimbursement. You will coordinate CAAs, care plans, and submissions within CMS and state guidelines, while collaborating with a multidisciplinary team.

Qualifications

• RN license in New York
• MDS experience in a Skilled Nursing Facility preferred
• Knowledge of PDPM, CMS regulations, and care planning processes
• Strong organizational, analytical, and communication skills
• Ability to work collaboratively with interdisciplinary teams

Responsibilities

• Coordinate and complete MDS assessments, CAAs, and care plans per CMS and state guidelines
• Ensure accuracy and timeliness of MDS submissions and supporting documentation
• Collaborate with interdisciplinary team to develop and implement individualized care plans
• Monitor compliance with PDPM requirements and regulatory standards
• Participate in Medicare meetings, care plan meetings, and clinical audits
• Review clinical documentation to support coding and reimbursement accuracy
• Assist with quality measures and performance improvement initiatives
• Stay current with regulatory updates and MDS best practices
