Pay Range:

$60,000 - $75,000

Job Responsibilities:

1. Provide direct, one-on-one medical care to infants, children, and adolescents in their homes according to the active care plan.
2. Conduct ongoing patient assessments and care.
3. Administer prescribed medications, treatments, and therapies.
4. Coordinate care and educate family members to improve outcomes.
5. Implement preventative measures to ensure quality of care.
