Job summary

Elevance Health is seeking an RN Clinician to serve as LTSS Service Coordinator in Marion County, IN. The role requires field visits to client sites and coordination of care across physical health, behavioral health, social services and long term supports.

You will develop, monitor and revise member care plans, perform assessments, and direct non‑RN staff to ensure services meet member needs. Hybrid work with occasional onsite requirements is expected.

Qualifications

• HS diploma or equivalent.
• Minimum of 3 years of experience working with individuals with chronic illnesses, co‑morbidities, or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active valid and unrestricted RN license in applicable state(s) required.
• May require state‑specified certification based on state law and/or contract.

Responsibilities

• Identify high risk members and coordinate care with the health care team.
• Manage members with chronic conditions to optimize health benefits utilization.
• Develop individual care plans based on member history and goals.
• Lead care plan development and oversee non‑RN clinicians on the case.
