Qualifications

• RN license active and unrestricted in state of practice.
• At least 2 years in health care and 1 year in care management/LTSS.
• Valid driver’s license and reliable transportation.
• Ability to work with diverse populations and independently with minimal supervision.

Responsibilities

• Completes comprehensive member assessments within regulated timelines.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians, and the care team.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Travel 25–40% as required by state/contractual requirements.
