Qualifications

• Current MA RN or LPN license.
• Proficiency in basic computer skills.
• Strong attention to detail.

Responsibilities

• Assess health care needs and develop an interdisciplinary plan of care.
• Address needs of medically complex and mentally compromised residents.
• Establish and maintain communications with staff, residents, families, and medical team.
• Administer medications and treatments per standards of practice.
• Document care and treatment accurately and timely; maintain records per requirements.
• Support MDS/MMQ processes with documentation.
• Monitor medication/treatment effects and provide feedback to the team.
• Lead nursing staff to deliver safe, effective care; educate residents on care plans.
