Wachusett Rehabilitation and Nursing Center is seeking an RN/LPN (Registered Nurse or Licensed Practical Nurse) to join the caregiving team in Gardner, MA. The role involves providing nursing care, administering medications, and assisting with diagnostic procedures to support residents and families within a family-friendly setting.

Qualifications

• RN or LPN license is required.
• 1 year of nursing experience preferred.

Responsibilities

• Provides nursing care to patients as assigned.
• Administers and records medications and treatments as assigned.
• Assists Physicians and others with diagnostic procedures and treatments.
• Reinforces health care instructions to patients and their families.
• Participates with team in planning, implementing and evaluating nursing care and patient activities.
• Assists in admission, transfer and discharge of patients.
• Observes and reports patients’ vital signs and condition to the appropriate staff.
• Records observations in patient charts.
• Maintains safe and comfortable environmental conditions for patients.
• Assists with unit management and can assume charge of the unit as assigned.
