ALIYA Healthcare Consulting is seeking a dedicated LPN/RN to join a skilled nursing facility where you will provide total nursing care to residents on the assigned unit and supervise team members. You will ensure compliance with facility standards and support high-quality resident care.

You will develop individualized care plans with the healthcare team, administer medications, document responses, and educate residents and families.

Qualifications

• Graduation from an accredited nursing program.
• Active IL LPN/RN license.
• No prior abuse convictions.
• Ability to follow policies and maintain safety.
• Strong communication and teamwork.
• Ability to care for diverse resident populations.
• Physically able to perform duties.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift.
• Participate in developing and implementing individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment promoting residents' physical, social, and psychological well-being.
• Educate residents and families on care plans and procedures.
• Monitor team performance and assist with development and corrective action.
• Delegate and ensure completion of nursing tasks to team members.
• Stay current with facility policies and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care and responses per facility guidelines.
• Act as a charge nurse: support resident care decisions and resolve issues.
• Follow all facility safety policies and procedures.
