Job summary

Nexus at Wood River is seeking a Licensed Practical Nurse (LPN) to provide high-quality resident care in a long-term care setting. You will assess resident needs, implement care plans, and work with a multidisciplinary team to achieve the best outcomes.

Responsibilities include administering medications, monitoring progress, collaborating with physicians, and educating residents and families. A valid Illinois LPN license and CPR certification are required, with prior long-term care experience

Qualifications

• Valid, current LPN license in Illinois in good standing.
• Current CPR certification.
• At least 1 year of professional experience in long-term care.
