Job summary

The Greens at Gastonia is seeking a Full-Time RN/LPN MDS Coordinator to join our team. We offer a $10,000 sign-on bonus and MDS training for the right candidate.

If you have strong long-term care experience and are interested in growing into an MDS role, we provide support from our Regional/Corporate MDS Team, no on-call shifts, and opportunities for advancement.

Qualifications

• Active, unrestricted RN or LPN license in North Carolina.
• Long-Term Care/Skilled Nursing experience required.
• Previous MDS experience preferred, but training available for the right candidate.

Responsibilities

• Coordinate and monitor completion of MDS assessments.
• Ensure assessments are accurate and timely.
• Assist with development and coordination of resident care plans.
• Facilitate interdisciplinary care plan conferences.
• Work with nursing, therapy, dietary, social services, and other departments.
• Ensure MDS submissions and validations are completed properly.
• Support compliance with Medicare, Medicaid, OBRA and regulatory requirements.
• Participate in QM meetings.
• Collaborate with Regional/Corporate MDS Team.

What You'll Do

As our MDS Coordinator, you will help coordinate the Resident Assessment Instrument (RAI) process and ensure the timely and accurate completion of MDS assessments, CAAs, and interdisciplinary care plans.

Responsibilities include:

• Coordinate and monitor completion of MDS assessments

• Ensure assessments are completed accurately and on time

• Assist with development and coordination of resident care plans

• Facilitate interdisciplinary care plan conferences

• Work closely with nursing, therapy, dietary, social services, and other departments

• Ensure MDS submissions and validations are completed appropriately

• Support compliance with Medicare, Medicaid, OBRA, and applicable regulatory requirements

• Participate in Quality Measure (QM) meetings

• Collaborate closely with our Regional/Corporate MDS Team

What We're Looking For

• Active, unrestricted RN or LPN license in North Carolina

• Long-Term Care/Skilled Nursing experience required

• Previous MDS experience preferred, but not required for the right candidate

• Strong clinical assessment and critical-thinking skills

• Excellent organization and attention to detail

• Strong written and verbal communication skills

• Ability to work collaboratively with an interdisciplinary team

• Knowledge of MDS/RAI, PDPM, Medicare/Medicaid reimbursement, Case-Mix, CAAs, and Quality Measures is a plus
