Job summary

Prime Healthcare is seeking an RN or LPN MDS Coordinator for a full-time role in long-term care. You will manage the RAI/MDS process, create care plans, and work with an interdisciplinary team to ensure accurate assessments and compliant documentation.

Ideal candidates have LTC experience and licensure in Tennessee, with a willingness to learn MDS. Training, development, and a supportive team environment are offered.

Qualifications

• Active, unrestricted RN or LPN license in Tennessee.
• Previous Long-Term Care / Skilled Nursing experience strongly preferred.
• Previous MDS experience is preferred, but training is available for the right LTC candidate.

Responsibilities

• Coordinate and oversee the RAI/MDS process, ensuring accuracy and on-time submission.
• Develop individualized resident care plans with short- and long-term goals.
• Collaborate with nursing, therapy, dietary, social services, physicians, residents and families.
• Monitor timely completion of resident assessments and MDS validation reports.
• Schedule and facilitate interdisciplinary care plan conferences and QM meetings.
• Maintain knowledge of Medicare, Medicaid, PDPM, OBRA, CAAs, Care Plans, Case-Mix, and Quality Measures.
