Willow Ridge Rehabilitation & Living Center in Rutherfordton, NC is seeking an experienced RN/LPN MDS Coordinator to own the MDS/RAI process, ensuring accurate assessments, regulatory compliance, and proper reimbursement.

Qualifications

• Active and unrestricted RN or LPN license.
• Minimum of 2 years of MDS experience in long-term care or skilled nursing.
• RAC-CT certification preferred and knowledge of Medicare reimbursement.

Responsibilities

• Oversee timely and accurate completion of MDS assessments.
• Manage the RAI/MDS process and ensure regulatory compliance.
• Review clinical documentation for accuracy and completeness.
• Coordinate interdisciplinary care planning and QM monitoring.
• Maintain required MDS documentation and ensure timely submissions.
