Position Overview

As the RN/LPN MDS Coordinator, you will oversee the full Resident Assessment Instrument (RAI) process, ensuring MDS assessments are completed accurately and on time while supporting individualized care planning, regulatory compliance, and reimbursement accuracy.

This position is a great fit for an experienced MDS professional who is organized, deadline-driven, detail-oriented, and committed to clinical excellence.

Key Responsibilities

• Coordinate and ensure timely and accurate completion of MDS assessments

• Oversee the RAI process and maintain compliance with applicable requirements

• Review clinical documentation for accuracy, completion, and regulatory compliance

• Manage MDS transmissions and validate required submissions

• Coordinate and participate in interdisciplinary care planning

• Monitor Quality Measures (QM) and identify opportunities for improvement

• Collaborate closely with nursing, therapy, providers, and facility leadership

• Ensure resident care plans accurately reflect individual needs, goals, and services

• Support accurate reimbursement through proper assessment and documentation practices

• Maintain required MDS documentation and meet established deadlines

Qualifications

• Active and unrestricted RN or LPN license

• Minimum of 2 years of MDS experience in long-term care or skilled nursing

• Strong knowledge of the MDS and RAI process

• Experience with care planning and clinical documentation

• Knowledge of Medicare reimbursement and applicable long-term care regulations

• RAC-CT certification preferred

• Strong organizational, communication, and time-management skills

• Ability to effectively manage multiple deadlines and priorities

• Team-oriented approach with a strong commitment to quality resident care
