Job summary

Willow Ridge Rehabilitation & Living Center in Rutherfordton, NC seeks an experienced RN/LPN MDS Coordinator to lead the MDS/RAI process, ensuring timely assessments, compliant documentation, and accurate reimbursement. You will partner with nursing, therapy, and leadership to optimize resident care and outcomes.

Qualifications

  • Active and unrestricted RN or LPN license.
  • Minimum of 2 years of MDS experience in long-term care or skilled nursing.
  • Strong knowledge of the MDS and RAI process.
  • Experience with care planning and clinical documentation.
  • Knowledge of Medicare reimbursement and applicable long-term care regulations.
  • RAC-CT certification preferred.
  • Strong organizational, communication, and time-management skills.
  • Ability to effectively manage multiple deadlines and priorities.
  • Team-oriented approach with a strong commitment to quality resident care.

Position Overview

As the RN/LPN MDS Coordinator, you will oversee the full Resident Assessment Instrument (RAI) process, ensuring MDS assessments are completed accurately and on time while supporting individualized care planning, regulatory compliance, and reimbursement accuracy.
