Job summary

Willow Valley Center for Nursing and Rehabilitation is seeking an experienced MDS Coordinator to join our team. As an RN or LPN, you will oversee the Resident Assessment Instrument (RAI) process and collaborate with the interdisciplinary team to ensure accurate, timely, and compliant assessments and care plans.

You will coordinate MDS, CAAs, and care plans with adherence to federal and state regulations, lead regular PPS and Medicare meetings, and support quality of care through thorough chart

Qualifications

• Current, active RN or LPN license in North Carolina.
• Minimum of 3 years of clinical experience in a healthcare setting; long-term care experience preferred.
• Previous MDS/RAI experience strongly preferred.
• Current CPR certification.
• Strong knowledge of federal and state regulations, guidelines, and requirements related to long-term care.
• Strong organizational, communication, and documentation skills.
• Ability to work effectively with residents, families, nursing staff, and the interdisciplinary team.
• Professionalism, personal integrity, attention to detail, and a commitment to quality resident care.

Responsibilities

• Oversee and coordinate the Resident Assessment Instrument (RAI) process with the interdisciplinary team.
• Coordinate and participate in the completion of the MDS, CAAs, and Care Plans in accordance with regulations.
• Develop and communicate MDS assessment schedules for all applicable payer sources.
• Communicate changes to the MDS assessment schedule to all appropriate interdisciplinary team members.
• Lead and/or participate in daily PPS meetings, weekly Medicare meetings, and month-end meetings.
• Gather and verify information required for MDS completion from medical records, bedside assessments, and interviews.
• Develop and monitor systems to ensure team members complete, date, and sign required assessments.
• Observe direct nursing care, review clinical documentation, and make recommendations.
• Assist with chart reviews and clinical audits to promote accuracy, compliance, and quality of care.
• Work collaboratively with nursing and team members to reflect resident changes in care plans.

What You'll Do

As the MDS Coordinator, you will oversee and coordinate the Resident Assessment Instrument (RAI) process while working closely with the interdisciplinary team to ensure accurate, timely, and compliant assessments and care plans.

What You'll Need
