Job summary

Cedar Crest at North Florida in Gainesville, FL is seeking a qualified RN/LPN MDS Coordinator to join our family-oriented team. You will oversee MDS processes, CAA’s and Care Plans while collaborating with facility staff to ensure regulatory compliance.

The role requires an active RN or LPN license with at least six months of MDS experience in a skilled nursing or rehabilitation setting. RAC-CT or RNAC certification is preferred, and we offer competitive pay and family health coverage.

Qualifications

• Must possess a current, unencumbered RN or LPN license in the state.
• Six months of experience as a MDS Coordinator in Skilled Nursing/Rehabilitation.
• RAC-CT or RNAC certification preferred.

Responsibilities

• Oversee coordination and completion of the MDS, CAA’s and Care Plan per regulations.
• Monitor Medicare/Managed Care residents with facility team members.
• Assist in implementing ideas to improve systems and processes.

Major Responsibilities

• Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA's and Care Plan) in accordance with current Federal and State Regulations.
• Monitor and document the management of the Medicare and Managed Care residents in collaboration with facility team members.
• Provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an RN/LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Six (6) months of experience as a MDS Coordinator.
• RAC-CT or RNAC preferred.
