HopeWest is seeking an RN/LPN Hospice Float nurse for the Grand Junction community. The role covers patient care across assigned regions, with visits, referrals, admissions, and telephone triage, while also participating in on-call rotation.

The position is cross-trained to support multiple teams and locations, enabling flexible nursing coverage and continuity of care for patients and families.

Qualifications

• Must have unrestricted RN or LPN license in Colorado.
• Must have valid Colorado driver’s license, reliable transportation and auto insurance.
• Minimum of 1 year experience, preferred in acute care setting.
• Premium pay for CHPN/WCC.

SUMMARY OF JOB RESPONSIBILITIES : The Float Nurse is responsible for providing patient care to assigned patients in their assigned regions. This may be providing a single visit, completing a referral or admission visit, providing telephone triage, and/or temporarily serving as a “primary nurse” to cover leaves and absences. This position will be cross trained to provide nursing services of individualized care to patients and their families in multiple teams/locations throughout HopeWest. This position also participates in on‑call rotation.
