HopeWest is seeking an RN/LPN to join our Hospice Float team in Grand Junction, CO. This part-time role (24 hours/week) provides flexible clinical support across hospice teams, ensuring continuity of care for patients and families.

You will perform assessments, plan care, manage symptoms and medications, and educate families while coordinating with the interdisciplinary team. The position requires an unrestricted Colorado RN license, a valid CO driver’s license, and at least 1 year of nursing

Qualifications

• Must have unrestricted RN license in Colorado
• Must have valid Colorado driver’s license, reliable transportation and auto insurance
• Compliance with training requirements for RN Float (PRN) Pool Program
• Minimum of 1 year of nursing experience
• 2 years in hospital, home health, oncology, critical care or hospice preferred

The Hospice Float RN/LPN provides flexible clinical support across hospice teams and service areas to ensure continuity of care for patients and families. This position delivers compassionate, patient-centered nursing care throughout the hospice journey, including patient assessments, care planning, symptom management, medication management, patient and family education, and coordination with the interdisciplinary team. The Hospice Float RN/LPN adapts to changing staffing and patient care needs, supports coverage for planned and unplanned absences, and maintains compliance with applicable regulations, clinical standards, and HopeWest policies and procedures. Requirements:

• Must have unrestricted RN license intheStateof Colorado
• Must have valid Colorado driver’s license, reliable transportation and auto insurance coverage maintained throughout employment.
• Compliance with training requirements is essential for participation in the RN Float (PRN) Pool Program.
• Minimum of 1year of nursing experience required.
• 2 years working in hospital, home health, oncology, critical care or hospice setting preferred.
