Job summary

HopeWest is seeking an RN/LPN for Hospice Float in Grand Junction, CO. Schedule: Full-Time, 3/12s, 7:00am–7:00pm with weekend coverage as needed. The role provides flexible clinical support across hospice teams to ensure continuity of care for patients and families.

You will perform patient assessments, care planning, symptom and medication management, and educate patients and families while coordinating with the interdisciplinary team. Competitive benefits offered by HopeWest.

Qualifications

• Must have unrestricted RN/LPN license in Colorado.
• Must have valid Colorado driver’s license, reliable transportation and auto insurance.
• Compliance with RN/LPN Float Pool training requirements.
• Minimum of 1 year of nursing experience.
• 2 years in hospital, home health, oncology, critical care or hospice preferred.
• Premium pay for CHPN/WCC.

Responsibilities

• Provide flexible clinical support across hospice teams and service areas to ensure continuity of care for patients and families.
• Deliver compassionate, patient-centered nursing care throughout the hospice journey, including assessments, care planning, symptom management, medication management, patient/family education, and coordination with the interdisciplinary team.
• Adapt to changing staffing and patient care needs; support coverage for planned/unplanned absences; maintain compliance with regulations and HopeWest policies.
