Job summary

American Medical Administrators LTC is seeking a compassionate Care Manager (RN/LPN) to coordinate resident care in a skilled nursing setting. You will partner with nursing leadership, CNAs, physicians, therapy, social services, and dietary teams to ensure seamless, individualized care plans and timely interventions throughout each resident’s stay.

Qualifications

• Current and unrestricted RN or LPN license.
• Nursing experience in skilled nursing, long-term care, rehabilitation, or a similar healthcare environment.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and interpersonal skills.
• Strong organizational and time-management abilities.
• Ability to effectively coordinate multiple residents, priorities, and clinical needs.
• Commitment to resident-centered care and interdisciplinary teamwork.

Responsibilities

• Coordinate and monitor the overall care of assigned residents.
• Serve as a clinical resource and point of communication for residents, families, providers, and members of the interdisciplinary team.
• Review resident clinical information and monitor for changes in condition, new orders, treatments, medications, and other care needs.
• Participate in resident care planning and interdisciplinary team meetings.
• Help ensure individualized care plans accurately reflect resident needs, goals, preferences, and current clinical status.
• Collaborate with Nurse Assessment Coordinator/MDS team to ensure resident assessments and documentation reflect the care provided.
• Monitor nursing documentation for accuracy, completeness, and appropriate follow-through.
• Coordinate with physicians, nurse practitioners, therapy, pharmacy, social services, dietary, and other healthcare partners.
• Assist with discharge planning and transitions of care.
• Advocate for residents promoting dignity, safety, and quality of life.
