Job summary

Holland Home in Grand Rapids, MI, seeks a dedicated RN or LPN for 2nd shift at Raybrook Manor. Deliver resident-centered care, perform health assessments, manage medications, and maintain clinical documentation in a 5-Star setting.

You will mentor CNAs and coordinate with physicians while supporting families with education and advocacy. Current Michigan RN/LPN license and CPR certification are required; prior geriatric care experience is preferred but not mandatory.

Qualifications

• Current Michigan RN or LPN license required.
• CPR certification required.
• Previous geriatric or long-term care experience preferred, but not required.
• Basic computer skills and ability to document within the electronic medical record (EMR).

Responsibilities

• Deliver compassionate, resident-centered nursing care.
• Perform assessments and monitor resident health status.
• Administer medications and treatments safely and accurately.
• Carry out physician orders and communicate clinical updates.
• Support, mentor, and supervise CNAs and other care team members.
• Assist with physician evaluations and procedures as needed.
• Maintain accurate and timely clinical documentation.
• Build meaningful relationships with residents and their families while providing education, support, and advocacy.
