You will deliver direct nursing care, supervise aides, document accurately, and contribute to care planning. A current RN license and strong communication skills are essential; long-term care experience and EHR proficiency are preferred.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role.
• Knowledge of Federal and State long-term care regulations.
• Strong communication and clinical judgment skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' condition and notify physicians of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and regulatory surveys.
• Perform other duties as assigned by leadership.
