Job summary

Signature HealthCARE of Terre Haute, a 176-bed facility, seeks an experienced RN to join its patient-centered team in Terre Haute, Indiana. We offer meaningful nursing care, strong teamwork, and opportunities to advance your career in a supportive environment.

Responsibilities include direct patient care, medication administration, ADLs, supervision of Nursing Assistants, and participation in quality assurance. A $10,000 sign-on bonus is available.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Familiarity with electronic health records (EHR) systems; proficiency with Microsoft Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
