Job summary

Signature HealthCARE of Erin is hiring an RN to join our 164-bed facility with a large therapy department and rehab-to-home program. You will provide direct nursing care, manage medication administration, supervise aides, and coordinate admissions and discharges.

Qualifications

• Current, active RN license in state of practice.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' status and notify physician of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
