Lead Resident Care Supervisor (RN) Overview

The Lead Resident Care Supervisor (RN) will be responsible for the clinical oversight and coordination of the NYS Licensed Home Care Services Agency (LHCSA)/Assisted Living Program (ALP), including UAS assessment for one or more locations, in conjunction with administrative staff and other service agencies.

Responsibilities

Responsible for coordinating ALP program requirements for eligible residents at one or more facilities. Travel to multiple sites will be required.

  • Ensure completion of a medical evaluation within 30 days prior to admission
  • Participate in pre-admission interviews and assessment screenings for prospective residents
  • Ensure completion of mental health evaluations for residents with a history of mental disability or when suggested by assessments
  • Complete a UAS-NY Community Assessment or PRI upon admission and every 6 months, as required
  • Create and maintain individualized care plans for ALP residents, including staff orientation and updates reflecting resident needs and preferences
  • Abide by all State and Federal Regulations
  • Maintain safe conditions for residents and staff
  • Participate in nursing on-call rotation
  • Demonstrate knowledge of policies and procedures and ability to reference them

Respond promptly to residents’ requests.

  • Perform duties according to service standards and goals
  • Provide excellent customer service to residents, staff, and visitors
  • Adapt to changing situations flexibly
  • Maintain confidentiality and privacy of resident information
  • Treat residents, staff, and visitors with respect and kindness
  • Respect and document residents’ rights and preferences
  • Manage lost/found articles in cooperation with administration

Supervise nursing care services and staff on all units during duty hours.

  • Conduct rounds, provide input on resident care, and serve as a resource for nursing staff
  • Monitor care standards and assist with staffing and resource issues
  • Delegate and assign care activities appropriately
  • Revise assignments based on census and acuity changes
  • Ensure completion of documentation and compliance with policies
  • Interpret policies to staff and observe performance, providing feedback
  • Perform accurate documentation of care and evaluations
  • Collaborate with other disciplines and educate residents as needed
  • Coordinate with physicians and update families accordingly
  • Participate in shift reports and ensure continuity of care

Promote safety and proper handling of controlled substances.

  • Distribute and destroy controlled substances following regulations
  • Follow safety procedures and promote safety programs
  • Assist with first aid and incident reports
  • Implement emergency procedures
  • Report significant events to management
  • Take responsibility for nursing services in the absence of the DON
  • Participate in on-call duties and adjust schedules as needed
  • Attend meetings and participate in Quality Improvement initiatives
  • Support ongoing staff education and professional development

Participate in facility on-call programs, work evenings/weekends if required, and utilize electronic systems as directed.

Maintain punctuality, adhere to dress code, and follow policies on electronic device use.

Qualifications

Educational Requirements

  • Graduate of an accredited School of Nursing
  • Valid NYS RN license in good standing
  • Minimum three years nursing experience, with one year in home care preferred
  • Willing to obtain NYS UAS assessor certification and PRI training
