Overview




To support the Mission of Helio Health, Inc., the CTI Team Leader will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a "critical time" transitional period in their lives. CTI is designed specifically to enhance the continuity of care during transition from hospital to community care.

CTI Team Leader

Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Critical Time Intervention Team Leader will oversee the new project implementation, be the point-person for engaging and providing education about the team and services provided to the community at-large. The Team Leader will oversee the team of two case managers. The Team Leader will facilitate weekly team meetings, monitor implementation, and be responsible for data collection and performance outcome reporting. The Team Leader will work on a Full-Time basis and using a hybrid work model, working in the community and out of Helio Health's Broome County region.

Schedule: Monday- Friday, 8:00am-4:30pm, with some evening on call responsibilities.

Pay Rate: $68,000.00 - $73,000 per year.

In addition to Helio Health's comprehensive benefits package, Program Directors are eligible for a Sign-On incentive up to $3,000-$8,000 depending upon licensure!






Responsibilities




    Maintains and provides supervision of program staff.
  • Implements Quality Improvement activities.
  • Fiscal program oversight and budget development.
  • Participates in Management and Administrative committees as assigned.
  • Assures program record documentation is in compliance with agency and regulatory standards.
  • Ensures that CTI workers' practice is consistent with the phase-specific activities and foci of the CTI GPD-CM model
  • Works to create relationships with key providers in the community
  • Other duties as assigned by supervisor.





Qualifications




*Physician: An individual currently licensed to practice medicine issued by NYSED;

*Physician Assistant: An individual currently registered to practice as a physician assistant issued by NYSED;

*Nurse Practitioner of Psychiatry: An individual currently certified to practice as a nurse practitioner issued by NYSED;

*Registered Nurse (RN): An individual currently licensed to practice as a registered professional nurse issued by NYSED;

*Psychologist: An individual currently licensed to practice as a psychologist issued by NYSED;

*Social Worker: An individual who is either currently a licensed clinical social worker

(LCSW) issued by NYSED or a licensed Master of Social Work (LMSW) practicing under the supervision of a psychiatrist, psychologist, or LCSW employed by the agency;

*Mental Health Counselor: An individual currently licensed to practice as a mental health counselor issued by NYSED;

*Marriage and Family Therapist: An individual currently licensed to practice as a marriage and family therapist issued by NYSED;

*Psychoanalyst: An individual currently licensed to practice as a psychoanalyst issued by NYSED; and

*Creative Arts Therapist: An individual currently licensed to practice as a creative arts therapist issued by NYSED.

3-5 years post licensure experience working with individuals who have mental health or co-occurring mental health substance use disorders. required"
