Job summary

The University of Texas Southwestern Medical Center in Dallas is seeking a CV RN Intake Coordinator who will manage patient referrals and admissions coordination in a fast-paced medical setting.

The ideal candidate holds an NLN-approved nursing program, a valid Texas license, BLS certification, and at least 3 years of intake or care coordination experience, with strong communication and collaboration across departments.

Qualifications

• 3 years of medical intake and/or care coordination experience.
• Experience in relationship development and managed care.

Responsibilities

• Receives and responds to inquiries and patient referrals.
• Determines appropriateness of referral for admission.
• Interfaces with hospital departments and physicians to facilitate admission.
• Evaluates financial reimbursement and obtains pre-certification.
• Determines resources to provide appropriate level of care.
• Provides overview of services during hospitalization to patients, families, payers.
• Notifies payor of admission and assigns initial length of stay.
• Tracks intake data and referral sources.
• Shares guidelines with staff on intake and referral processes.
• Communicates referral information to rehab unit.
• Accepts additional responsibilities and organizes time.
• Performs other duties as assigned.
