Job summary

Civic Minds is seeking an RN Intake Manager to coordinate the intake process for patients referred to home care services. You will act as the primary liaison among hospitals, referral sources, patients, families, and internal teams to ensure a seamless transition from hospital to home.

The role requires NYS RN license, CHHA experience, and proficiency with Allscripts and Home Care Home Base, along with excellent communication and organizational skills in a high-pressure environment.

Qualifications

• NYS Registered Nurse license is required.
• Experience with Allscripts is required.
• CHHA experience is required.
• Experience with Home Care Home Base is required.
• Computer literacy is required.
• Excellent communication skills are essential.
• Highly organized with strong attention to detail.
• Experience multitasking in a high-pressure environment.

Responsibilities

• Coordinate the intake process for patients referred to home care services.
• Serve as the primary liaison between hospitals, referral sources, patients, families, and internal teams.
• Review referrals and coordinate appropriate home care services.
• Facilitate timely admissions to home care.
• Provide exceptional customer service by addressing inquiries and concerns with professionalism, compassion, and efficiency.
