Job summary

Civic Minds is seeking an RN Intake Manager to coordinate the intake process for patients referred to home care services. You will act as the primary liaison among hospitals, referral sources, patients, families, and internal teams to ensure a seamless transition from hospital to home.

The role requires NYS RN license, CHHA experience, and proficiency with Allscripts and Home Care Home Base, along with excellent communication and organizational skills in a high-pressure environment.

• Computer literacy is required.

Serving as the primary liaison between hospitals, referral sources, patients, families, and internal teams, this role ensures a seamless transition from hospital to home. Responsibilities include reviewing referrals, coordinating appropriate home care services, facilitating timely admissions, and providing exceptional customer service by addressing patient inquiries and concerns with professionalism, compassion, and efficiency.
