Americare is seeking an Intake Coordinator to join our home health team. A Registered Nurse will take referrals and process appropriate home care services, while maintaining clear communication with referral sources and internal Americare departments.

This role emphasizes a positive customer service approach and attention to detail, with opportunities for advancement and a comprehensive benefits package.

Qualifications

• Current NYS RN License.
• Allscripts is a MUST.
• CHHA experience is a MUST.
• Computer literacy; experience with Home Care Home Base is a MUST.
• Excellent communication skills.
• Highly organized, with strong attention to detail.

Responsibilities

• Process referrals and appropriate home care services.
