Job summary

Americare is seeking a diligent Intake Coordinator RN to join our remote/full-time team. You will take referrals, assess eligibility, and process appropriate home care services while maintaining clear communication with referral sources and internal departments.

As a trusted RN, you will promote Americare through exceptional customer service, ensure accurate documentation, and collaborate with management to support reliable access to home care for our clients across New York.

Qualifications

• Current NYS RN license.
• Allscripts proficiency required.
• CHHA experience required.
• Computer literacy; experience with Home Care Home Base required.
• Excellent communication skills.
• Highly organized with strong attention to detail.

Responsibilities
