Qualifications

• Current NYS RN license.
• Allscripts proficiency required.
• CHHA experience required.
• Computer literacy; experience with Home Care Home Base required.
• Excellent communication skills.
• Highly organized with strong attention to detail.

Responsibilities

• Take referrals and process appropriate home care services.
