Job summary

Qualifications

• Bachelor's degree in nursing required.
• RN licensure in VA/NC/Compact within stated region; BLS within 90 days of hire.
• Case management certification (ACM/CCM/CCCTM/CMAC/CMGT-BC) within one year of eligibility.

Responsibilities

• Coordinate and facilitate comprehensive care management across the patient care continuum.
• Focus on patient transitions to appropriate levels of care and clinical appropriateness; monitor LOS.
• Develop and implement care plans addressing physical, mental, and psychosocial needs of inpatients.
