Job summary
Option Care Health, Inc. is hiring a Clinical Transition Specialist in California. You will sell home infusion services to discharging patients, educate patients and care teams, and coordinate transitions to home or alternate sites. You will build strong relationships with referral sources and work with Account Managers to grow referrals.
Minimum 2 years in healthcare and a licensed RN or Pharmacist are required. Travel to partner facilities and hospitals will be involved as part of this role.
Qualifications
- Licensed RN or Pharmacist in the state of practice.
- Minimum 2 years in healthcare or related industry.
- Proven ability to develop relationships with referrals and vendors.
- Strong communication and presentation skills.
Responsibilities
- Proactively grows relationships with referral sources to increase sales and patient starts.
- Educates patients, families, and facility staff about services and products.
- Coordinate care transition to home or ATS with internal and external partners.
- Ensure accurate pre-admission information and signoffs.
- Partner with Account Manager to execute area growth plans.
- Maintain confidentiality and comply with legal guidelines.
